The United Kingdom Marine Accident Investigation Branch issued its investigation report on the Aug. 3 grounding of a container ship at Pendeen in Cornwall, England.

According to the report, at 3:23 a.m., while on passage from Cork, Ireland, to Rotterdam, Netherlands, the intoxicated master relieved the second officer as officer of the watch. No lookout was posted. The master soon fell asleep. The vessel continued on for two hours until the grounding.
